Tidelands Health HAS transitioned to MyChart
Tidelands Health has transitioned to the industry-leading Epic electronic medical record platform.
Going forward, you'll access your Tidelands Health medical records through MyChart, available in the all-new My Tidelands Health app or via tidelandshealth.org. As part of this process, we'll migrate your past three years of lab results and past five years of imaging results.
If you need access to medical records not available in MyChart, you can always request them online, via mail or via fax pursuant to the instructions below.

Authorization for release of medical information.
Your medical records belong to the hospital, yet the information contained in your records belongs to you.
Your medical records can be requested online, by mail or by fax. Please see information for each below.
Request Your Medical Records, Reports and Images Online
Use the button below to submit a request through our online form, or download the form on the same page to complete and submit to Tidelands Health by mail or fax. All requests for radiology reports and images can now also be obtained online by clicking the button below. After submitting your request, you will receive a QR code that can be scanned to download your images at your convenience.

Important Additional Notes
- All requesters must provide proof of identification or documented proof of legal relationship – personal representative, patient advocate, executor, trustee or guardian. Those with a legal relationship with the patient as listed will need to provide proof of identification.
- To give access to outside medical records, you will need to authorize release from your current health care provider(s). Please complete the form and send to your current provider for processing.
- Medical records requests are processed in the order they are received and will be delivered to you as quickly as possible. For urgent needs, please contact the Tidelands Health medical records department.
- Depending on the purpose of your request, there may be a fee for a copy of the records. You will be advised if there is a fee when your request is submitted and again before it is completed.
- Tidelands Health partners with MRO to process medical records requests. For help with a request, please contact MRO at 610-994-7500, option 1, Monday-Thursday, 8:30 a.m.-8 p.m. EST and Friday, 8:30 a.m.-6 p.m. EST.
